Job Description:

This role functions autonomously to deliver high-quality physical therapy services either through referral or direct-to-consumer care. The Physical Therapist will evaluate patients, develop treatment plans, assess progress, and ensure that patient needs are met through timely access to care, excellent patient experience, and adherence to quality care metrics. This position also supports program development and enhancement to meet evolving patient and community needs.

 

Responsibilities:

  • Perform evaluations and design treatment programs to meet patient-specific goals.
  • Monitor patient response to therapy and adjust plans as needed.
  • Maintain accurate and compliant patient documentation.
  • Meet established productivity standards, including access to care, patient experience, and quality benchmarks.
  • Assist in developing and enhancing programs to meet community needs.
  • Tailor care approaches the patient population, ensuring respectful communication and privacy.
  • Perform other related duties as assigned.

Requirements:

  • Education: Bachelor’s degree in Physical Therapy (required).
  • Experience: Minimum of 2 years of professional practice.
  • Licensure: Current Physical Therapy license in the state of practice.
  • Routine exposure to blood, body fluids, and tissues.
  • Work directly with patients and/or specimens with potential communicable diseases.
  • Potential occupational exposure to hazardous medications or waste, depending on role duties.

  • Maintain knowledge of current federal, state, and local regulations, as well as accreditation standards.
  • Adhere to the employer’s Compliance & Privacy Program and Standards of Conduct.
